Physical Therapist / Field Clinician

Job Title:

Physical Therapist / Field Clinician

Location:

Baltimore County & City - Travel required for community activities, office meetings, and patient visits.

Position Summary:

Responsible for administering to clients, according to physician’s orders, various

evaluations and therapeutic procedures and maintains agency records on clients.

Compensation:

Duties and Responsibilities:

·       Conducts the initial assessment when therapy is the only ordered service, identifying the patient’s immediate care needs and determining Medicare eligibility, including homebound status, when applicable.

 

·       Initial comprehensive assessment visit is completed within 48 hours of referral, or within 48 hours of the patient's return home. PT obtains physician ordered SOC if the patient requests delay. Documentation must be provided if the patient requests a delay, including an order and communication note, and the required documentation should be completed within a 24-hour timeframe.

 

·       Collaborates with the patient’s provider to assess functional status using appropriate clinical tools and tests contributing to the development and revision of the plan of care.

 

·       Treats clients to relieve pain, develop and restore function, and maintain maximum performance using appropriate physical therapy treatments, modalities, and procedures as stated in the physical therapy plan of care.

 

·       Observes, records, and reports the client’s reaction to treatment and any change in the client’s condition to the physician and/or clinical manager and other staff involved.

 

·       Provides education, supervision, and counseling to patients and their families on disease management, home safety, fall prevention, infection control, and mobility strategies. Instructs on the safe and effective use of assistive devices, prosthetics, orthotics, and therapeutic interventions based on the patient’s condition. Assesses barriers to learning and uses MSA standard zone tools based on patient diagnosis to support individualized teaching and care planning.

 

·       Completes clinical documentation accurately and in real time during the patient visit whenever possible to ensure timely, complete, and compliant records. If unable to complete during the visit, notifies the supervisor and submits documentation within 24 hours.

 

·       Provides Age-friendly care for each patient according to the established plan of care to include the 4M’s (What Matters Most, Medication, Mentation, Mobility) with treatment and services.

 

·       Responsible for managing the patient’s episode of care from start of care (SOC) through discharge. This includes developing and implementing individualized care plans, monitoring visit frequency, and coordinating services to ensure patient needs are met. The PT collaborates with the interdisciplinary team throughout the episode to promote continuity of care, support accurate recertification decisions, and ensure appropriate and timely discharge planning. Patient goals and progress are routinely evaluated to guide clinical decision-making and promote optimal outcomes.

 

·       Evaluates and re-evaluates the patient’s rehabilitation needs at required intervals (at least every 30 days), initiating and revising the plan of care as needed.

·       Compares results of standardized tests at each re-evaluation to gauge the effectiveness of therapy, assess progress, and determine continued service needs.

·       Ensures the plan of care addresses all identified problems or includes appropriate clinical rationale when exclusions apply. Assesses ongoing care needs and updates the plan accordingly.

 

·       Performs medication review and reconciliation for all medications the patient is currently using in order to identify any potential adverse effects and drug reactions, to include ineffective drug therapy, significant side effects, significant drug interactions, duplicate drug therapy, and noncompliance with drug therapy.

 

·       Med Rec reviews all prescription and non-prescription medications (e.g. over the counter medications, herbal and other alternative treatment) following MSA policy time points. Provides education to patient, family, and caregiver in a manner in which they can understand.

 

·       Initiates diagnostic, preventive, and rehabilitative procedures as appropriate to the client’s care and safety. Makes referrals to other disciplines as indicated by the needs of the client or documents rationale for not doing so.

 

·       Supervises and teaches the home health aide in conjunction with the skilled nurse or in place of the skilled nurse when nursing not in the case. Assesses the client’s need for unskilled care every 14 days and revises the plan of care as appropriate.

 

·       Communicates plan of care changes to the aide and scheduler as often as necessary.

 

·       Monitors and evaluates PTA performance in the provision of services, provision of treatments, patient education, communication with the PT, and data collection regarding the patient’s status and health needs. Supervision occurs every 30 days or more frequently if needed per state guidelines.

 

·       Attends patient interdisciplinary team conferences prepared to discuss current patient status, lack of progress towards achieving goals, outstanding goals and interventions.

 

·       Needs identified requiring changes are relayed to physician/allowed practitioner to obtain updated orders.

 

·       Actively participates in the agency’s quality improvement program.

 

·       Follows all federal, state, and accrediting agency standards.

 

·       Stays current with industry standards and best practices, participating in ins-ervices, continuing education, required MSA annual training, and assigned education as needed to support professional development.

 

·       Collaborates with clinical leadership to discuss care coordination and discharge planning prior to finalizing the plan of care, notifying the physician or allowed practitioner, and/or initiating patient discharge from services.

 

·       Adheres to all agency policies and procedures related to service delivery, documentation, and interdisciplinary care coordination.

 

·       Contributes to overall team success by supporting agency operations and fulfilling related duties as needed.

Duties and Responsibilities:

Job Description: A CDL Driver is required to drive tractor trailer with track hoe / equipment from job sites as part of the pipeline and excavation projects team

 

Ensure a safe work environment in compliance with all safety policies and procedures utilizing the appropriate tools and equipment for the task

 

Thorough knowledge of the hazards associated with driving trucks at/near trenches and all the additional safety precautions

 

Operate excavation equipment that is being utilized to perform a task, Excavator, Loader, etc.

 

Assure all paperwork is filled out and handed in daily such as dump forms, material tickets and DOT pre-trips inspections

 

Must maintain the cleanliness of all company owned and rented vehicles/equipment
